When rebuilding doors, how many people replace all the paper/cardboard/foam that came originally with the car? I am talking about all the stuff that goes on the door before you hang the upper and lower panels...Want to know what folks are doing and how much sound deadening it actually accomplishes....

The paper water barrier could (economically) be replaced with some plastic sheeting/drop cloth, just cut to fit and use some tape. That part doesn't really make much of a difference as far as sound deadening. It doesn't sound like your car is equipped with the jute pad in the doors, but that definitely does make a difference and what smells bad over the years as the original paper water barrier doesn't do anything but hold moisture. Adding some Evercoat "Q-Pads" or some variant of Dynomat on the inside of the door skin makes a difference.
