Diagnose: Unknown Issue

I just had a CVF serpentine belt installed on my small block olds 350. Everything works fine BUT…when I put it in REVERSE the power steering line always pop off and I lose power steering fluid and the ability to steer and control the vehicle. If you look closely at the attached pics, you can see where the line has completely blown off (TWICE). No one seems to know the cause so I’m going to ask the masses for help diagnosing this issue. Please advise:

If you are blowing the hose right out of the AN hose end fitting then I would say you are exceeding the pressure limit for that set up.
I assembled every AN hose on both my Gbodys, except for the high pressure side of my power steering pump which I had a hydraulic hose shop make for me to avoid any issues.
