Have you tried to tighten up the screws? You can see them in the pic below...

I believe there is also a plastic sleeve in there...perhaps your's is worn or maybe cracked or broken?
 
Mine don't have the screws?

Perhaps yours only have the plastic sleeves and they are worn out? I can honestly say that I've never had a visor apart. I did find once that I was able to turn the chrome pin (that goes into the visor) into the mounting base and tighten it up?..visor removed from car of course. Maybe someone with more experience with the visors will chime in.
 
They should all have the screws and plastic sleeves. Maybe someone recovered them and covered over the screws.

I'll investigate further. Good possibility, I don't remember if these were recovered or I may have ordered newer ones recovered?
