I don't have any parts or part #s for you, but I will tell you one thing about the 3rd brake light housing.
(if you didn't know already).
Don't use an 1156 bulb in the 3rd brake light housing... it's way too hot, and that's what caused mine to start cracking and crumbling. Use a 1003(same base as 1156). I've seen the 1156 listed as the replacement bulb for that location on some sites, and while it's plenty bright (almost blinding at close range), it's just too hot for the plastic. When you stand back and look at it with a 1003, the 3rd brake light looks to be the the same brightness as the tail lights.
Thankfully it was mostly just the socket that was cooked on mine, i trimmed out a roughly circular opening where the original plastic socket was, and clipped in a metal socket with a large flange that had some "fingers" pointing in the direction of the bulb. I used some silicone to fill up the gaps between the plastic housing and the metal socket. A little bit hacky, but it works. I believe the package said it fit some Chrysler applications... probably others as well.

I remember the fingers being a little longer, maybe more curved too... but that's pretty much what it looks like.
