Steering Coupler Disc Question

Ok so i just went and bought the coupler disc but dont really know how it goes on because it has brackets, screws, nuts, and studs. And since the one that was in there is gone I have nothing to go by. Just want to get some advice on how it goes on. I can probably figure it out but really dont want to get down there without any kind of guidance. Thanks

2 of the bolts go to the flange on the steering shaft, 2 are for the flange on the steering box.
