New door hinge issue

Hey guys, I just installed new door hinges on my driver side door to try and solve an alignment issue. Just as I get the door aligned to my liking I see that the bushing on the upper hinge is walking itself off the pin. The ramp that rides on it also seems to be incredibly steep or something, making a heavy clicking noise. I only have to close the door 6 or 8 times and the bushing is about fallen off the pin. I was able to get it back on but the plastic inner sleeve is now cracked. It continues to walk itself off. Major bummer since it took a couple hours to get it aligned perfectly. Thoughts?

Is the pin staked at the upper portion, above the top of the hinge stamping?
Yes it’s staked.

Another thing I noticed, there is play at the serrated bushing around the outside diameter. It will egg out the hole in short time.

I think what’s going on with the original issue is the ramp that the roller rides on is cut too aggressively. When the door is open it sits in a low spot, but it’s too aggressive. This is putting stress on the roller and pushing it down off the pin. I compared it to the hinge I removed but I didn’t take a pic.

I cut the end off and drilled and tapped it so a bolt holds it in on that side. The pin is bigger than the existing hole.

Oh gotcha. So like a 1/4-20 bolt or maybe smaller?
