Does the reverse rotation water pump change just that? Or is the coolant flowing through the block in a different direction?
 
Coolant flows the same way, the impeller vanes are reversed so the coolant still flows like it's supposed to. The setup you saw was probably on a full size truck if it's steel. Avoid these like the plague, I had issues with the steering pump pulley hitting the steering box (4-bolt steering cover, round snap-ring cover clears fine). My setup is off a 4.3-powered S-Series, anything from the late '80s on up in those trucks should fit alright. So long as it's aluminum you should be ok.
 
By 69' 202 heads, are you meaning the double hump heads without any bolt holes in the front? If so I've heard of people drilling them out and tapping them to put serpentines on.

I've ran the 3rd gen F-body setup on my car for almost 6 months (wow it's been a while!) with only one bolt on the passenger side with no issues at all. I even removed the smog pump and used the alternate belt routing with the longer belt and it works great.

On the 4.3 S10 ones, you NEED to make sure that the alternator is on the driver side of the engine and not the passenger. The newer style ones 98+ will not fit a SBC with a mechanical fuel pump.
