Idle problem

I took this video to show an idle problem I've had with it for years now. It hadn't bothered me bad enough to figure it out since I had another vehicle for a daily driver. Well circumstances have made me pull the Cutlass out as a daily driver right now and I need to see if I can fix this to make it reliable... It really only does it when it's hot and only when in gear...

Can you explain what exactly it is doing? To me, it looks like it is running smooth but has a knock.
 
x3 on the knock. seems to be the drivers side.

idk about the knock but i would do a compression check on ur engine and a leak down too see whats up

Compression test tests good... Don't have the gauges to do a leak down test... If I had the money I'd like to do it though..

i know where i live kragen oreilly lends u tools....a compression check is good but wont let u know if its holding it so if u can do a leak down do that which will be the best thing to do you also need a compressor
 
start the car and kill one spark plug wire at time....if the knock goes away you'll know which cylinder it's coming from and it's probably a rod bearing. The other thing you can check is the flex plate, I've had them break and it can sound just like a bearing knock

I have the compressor... It's just the gauge set for the test I'm lacking...
