Would 285/40ZR18 tires rub on the back of my 1986 Regal, and would 5x4.5/114.3 mm bolt pattern fit on the 5x4.53/115 mm bolt pattern which is on my Regal?

Depends on the wheels' backspacing/offset, and no. The correct bolt pattern for g-bodies is 5x4.75 or 5x120.65.

G-bodies have 5x4.75 wheel bolt patterns not 5x4.5.

Easiest way to see if they will fit is to measure your car and find out. However a frame notch is unlikely to be needed.

Go to turbobuick.com and look up their sticky wheel and tire set ups under their brakes, suspension, tires, and wheel section......there are actual photos of different wheel combos with the sizes that fit right tucked or fitted with re fabbing the frame.....
